Registered Nurse (RN) Care Coordinator

Located in the heart of Charlottesville, VA, this on-site role allows you to support a dedicated patient population while working alongside a collaborative, interdisciplinary care team. As an RN Care Coordinator at CareATC, you’ll serve as a central clinical resource for patients managing chronic conditions—providing care coordination, patient education, and proactive outreach to support prevention, treatment adherence, and long-term health outcomes. This role is ideal for nurses who want to step away from traditional bedside or volume-driven care and focus instead on clinical assessment, care planning, and relationship-based support. What You’ll Do Conduct clinical assessments and ongoing patient outreach Support chronic disease management and care coordination programs Educate and empower patients to self-manage their health Coordinate care across providers, clinics, and services Document care plans and patient interactions in the EMR Participate in interdisciplinary team meetings What You’ll Bring Bachelor’s degree in nursing Active RN license in Virginia.
