- Conduct thorough evaluations of patients in inpatient or outpatient contexts and craft personalized treatment plans to meet their unique needs
- Deliver targeted physical therapy interventions that focus on enhancing mobility, building strength, improving balance, and overall functionality
- Work hand-in-hand with a multi-disciplinary team including nurses, doctors, and rehabilitation professionals to ensure optimal patient outcomes
- Provide education to patients and their families about therapeutic objectives, planning for discharge, and exercises to pursue at home
- Maintain detailed documentation of evaluations, treatments, and patient progress in line with established facility and regulatory standards
- Engage in care team discussions and interdisciplinary meetings to promote synchronized and comprehensive patient care
- Doctorate in Physical Therapy (DPT) or an acceptable equivalent from an accredited institution
- Valid state licensure as a Physical Therapist
- CPR certification preferred
- Additional specialized certifications (i.e., neurological or geriatric therapy) are advantageous but not mandatory
- Significant experience is highly preferred
- Strong capability to treat patients dealing with complex medical and mobility complications
- Exceptional communication skills and team collaboration
- Proficient with digital charting and clinical documentation systems
