The Home Health Physical Therapist provides specialized physical therapy services to patients in their homes, focusing on personalized care with a manageable caseload of 1-5 patients per day. The role involves moderate travel, up to 30 miles, to ensure patient access to quality rehabilitation. Collaboration with a multidisciplinary healthcare team, including nurses, physicians, occupational therapists, and social workers, is essential to develop and implement effective treatment plans. The therapist utilizes electronic health record systems like WellSky and CareStitch for accurate documentation and coordination of care. Availability at least two days a week and willingness to work alongside a Physical Therapist Assistant are key components of this position.

 

Responsibilities

• Assess patient physical condition and functional mobility
• Develop individualized treatment plans
• Implement therapeutic exercises and mobility training
• Manage pain and educate patients on therapy techniques
• Document patient progress using EHR systems (WellSky, CareStitch)
• Evaluate home safety and recommend appropriate equipment
• Collaborate effectively with healthcare team members
• Coordinate care with Physical Therapist Assistant as needed

 

Required Qualifications

• 1+ years experience in physical therapy
• Bachelor's or Master's degree in Physical Therapy
• Licensed Physical Therapist in the relevant state

• Strong skills in therapeutic exercise, mobility training, and pain management
• Effective patient education and documentation practices
