Care Coordinator – Full Time The Mental Health Association of Columbia-Greene Counties Inc. is hiring for three full-time Care Coordinator positions to assist Medicaid‑eligible individuals in coordinating behavioral and/or physical health care across Columbia and Greene counties. A $2,500 sign‑on bonus is offered, paid in two installments: the first issued with the first paycheck, and the second after six months of successful employment. Job Duties Coordinate with service providers and health plans to secure necessary care for members. Conduct fieldwork (≈75% of time) including traveling to clients’ homes and community sites to perform assessments and manage ongoing care. Provide transportation to clients when necessary. Connect and refer clients to appropriate services supporting their care plan and treatment goals (medical/behavioral health care, patient education, self‑help recovery and self‑management). Conduct case reviews with interdisciplinary team to monitor and evaluate client status and needs. Advocate for services and assist with scheduling of needed services. Collaborate with treating clinicians to ensure services are delivered and to address changes in medical treatment or conditions. Monitor and support clients in attending scheduled medical, behavioral, and social service appointments. Provide crisis intervention when needed. Revise care plans and goals as necessary. Qualifications Bachelor’s degree in a social service or counseling discipline (e.g., social work, psychology, nursing, rehabilitation, community mental health) or a Registered Nurse with a bachelor’s degree and at least two years’ experience in direct mental health services or linking clients to community resources. Exceptions to the bachelor’s degree may be made at the discretion of the program supervisor based on experience. Experience in direct service in the mental health or social service field and linking clients to community services. Reliable transportation to travel to client locations and other work sites when an agency vehicle is unavailable. Proficiency or willingness to learn online technology systems. Ability to work with a diverse population of urban, suburban and rural clients from varying socioeconomic backgrounds. Excellent oral and written communication skills. Familiarity with Greene County and knowledge of managed care is a plus.
