Lead Care Manager (ECM) - Hybrid Role

This is a hybrid position (2 days remote, 3 days in the field) , giving you the flexibility to work from home while building strong, in-person connections with members across Low Desert regions. Key Responsibilities

* Serve as the primary point of contact for members and coordinate their care

* Develop and manage personalized care plans with members and providers

* Collaborate with healthcare teams and community resources

* Support members through education, advocacy, and engagement

* Identify and address gaps in care

* Maintain accurate, timely documentation

Requirements

* Experience in care or case management (community-based preferred)

* Knowledge of Medi-Cal and ECM strongly preferred

* Strong communication and interpersonal skills

* Ability to work in a team-oriented environment

* Willingness to travel within Orange County

* Bilingual (English/Spanish) preferred
