Aftercare Case Manager (Full-time) Average hourly rate is $19.23 (including wage, incentives, bonuses, overtime, shift differential, etc.). What You’ll Need A bachelor’s degree in human services. A valid driver’s license. A willingness to learn—we’ll provide all the training you need! A Day in the Life Focus on risk management and help families maintain stability during the aftercare period. Provide crisis intervention and work with public and private agencies offering supportive services. Maintain accurate documentation and reports. Coordinate community and educational services for families. Meet with families in-person to support reunification and resolve any concerns.
