Physical Therapist (PT)

Facility: Good Samaritan Hospital Job Type: Full Time Overview Physical Therapist (PT) joining the team, dedicated to improving community health. Responsibilities Assumes responsibility for a designated group of patients and performs evaluations, modalities, and treatments per hospital policies. Assists patients to reach maximum performance and level of function while teaching self‑management within limits. Provides care under Physical Therapist Assistants and therapy techs; may orient staff and supervise in their absence. Assesses patients’ needs, initiates appropriate PT interventions, and collaborates with interdisciplinary team. Observes outcomes, evaluates treatment efficacy, and recommends changes to physician as needed. Performs assessments and reassessments, documents QI scoring, and formulates patient‑specific care plans. Reports patient progress and discharge barriers at team conferences. Provides patient safety‑focused care, reduces risk of errors. Develops teaching plans, evaluates effectiveness, involving families as appropriate. Documents all aspects of patient care accurately. Demonstrates competency in PT modalities. Qualifications Current licensure as a Physical Therapist in the state where the hospital is located. Inpatient rehabilitation experience preferred.
