In-Home Nurse Practitioner or Physician Assistant

Signify Health is seeking a part-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for primarily Medicare and vulnerable populations in their homes. You will conduct detailed histories and physicals, review medications, assess social determinants of health, and close care gaps using digital tools and evidence-based guidelines. You’ll document findings in our platform, collaborate with care teams and health plans, and provide education that supports value-based, patient-centered care in a flexible, mission-driven environment.
