UCP Seguin of Greater Chicago Education Transition Support SpecialistAnnual salary $43,680About the Role:We are seeking a passionate Education & Transition Support Coordinator to advocate for children and youth in foster care, ensuring access to quality education and successful transitions to adulthood. This role partners with case managers, caregivers, schools, and community agencies to support educational outcomes and compliance with state and federal regulations.Key Responsibilities:Serve as the primary education liaison, ensuring timely school enrollment, attendance, and appropriate educational servicesAdvocate for children's educational rights, including special education and early intervention servicesCollaborate with case managers and caregivers to support academic success and resolve school-related challengesMonitor educational progress, maintain accurate records, and ensure compliance with agency and regulatory standardsProvide training and guidance to staff and caregivers on education policies, advocacy, and available resourcesSupport transition planning for youth (age 14+) including post-secondary education, employment, and life skillsBuild and maintain relationships with schools, districts, and external partnersQualifications:Bachelor 's degree in Education, Social Work, or related field preferredMinimum 2 years of teaching, special education, or child welfare experienceStrong knowledge of education systems, child welfare, and advocacy practicesExcellent communication, organizational, and problem-solving skillsValid driver's license and ability to travel locallyWhy Join Us:You'll make a meaningful impact by empowering children and families while working in a collaborative, mission-driven environment focused on education, advocacy, and long-term success.
