Working Title: PRN Registered Nurse Case Manager Classification Title: Registered Nurse Division: Client Health Services Program: Family Health Services Reports to: WIC/FHS Program Manager 1 FLSA status: Non-Exempt: PRN (as needed) Wage Range: $34.52 to $55.60 hourly including 10% PRN Premium (based on number of years of transferrable experience and internal equity) RiverStone Health serves the Yellowstone County community and south-central Montana for nearly 50 years and provides a range of health services including medical, dental, behavioral health, home care, hospice, public health, and education. Job Summary KidsFirst Registered Nurse Case Manager serves Yellowstone County children and their foster families, coordinating care across systems that impact children in foster care, including the foster care system, healthcare, and family caregivers, to meet healthcare needs. Essential Functions / Major Duties and Responsibilities Nursing Services 60% Welcomes client/family, establishes rapport, and answers questions about treatments, immunizations, health practices, prevention, or care plans. Provides case management and health education services to clients and families in community, homes, or specialized programs. Conducts initial client evaluation visit and assessment, including medical histories, and performs regular follow‑up assessments, adjusting treatments as necessary. Maintains documentation such as clinical and progress notes, vaccine records, treatment plans, and other applicable records. Informs the primary medical provider of any changes in client condition or needs and coordinates treatment modifications. Assesses and determines physical, social, and environmental needs and develops a treatment plan with other team members. Provides skilled nursing assessment, facilitates health‑care plans, access to immunizations, and treatments. Communicates and coordinates with appropriate partners to maximize inter‑agency effectiveness and participates in quality improvement projects. Reports promptly to supervisor and designated staff about progress, problems, and improvement ideas. Attends meetings, trainings, and reviews materials to stay current on practices and developments. Uses nursing services to meet public‑health activity requirements as needed. KidsFirst Services 35% Conducts unaccompanied home visits at client/family residences, foster homes, or public settings. Provides ongoing emotional support and advocates for clients with community support services. Assesses physical, social, and environmental needs. Develops a treatment plan with team members. Completes initial evaluation and obtains medical histories. Prepares clinical and progress notes and maintains records. Coordinates agency services with other community agencies and makes referrals. Acts as liaison between client and other agencies when referred elsewhere. Company cell phone will be provided for client communication purposes. Non‑Essential Functions / Other Duties 5% Perform other duties as assigned to support the organization’s mission and goals. Education and Experience Minimum Qualifications Active Registered Nurse license. 3–5 years of patient‑care experience. American Heart Association Basic Life Support (BLS) certification. Any combination of experience and training that provides the equivalent scope of knowledge, skills, and abilities required. Preferred Qualifications Bachelor’s degree in nursing. Maternal or child health experience. Experience working with underserved populations. Certificates, Licenses, Registrations Current Registered Nurse license in Montana. Valid Montana driver’s license and vehicle insurance. BLS certificate. Knowledge, Skills, and Abilities Knowledge of customer and personal services principles, quality standards, and satisfaction evaluation. Knowledge of nursing techniques and their relationship to medical and surgical practice. Knowledge of public health principles, preventive measures, and community resources. Knowledge of care skills appropriate to clients served. Understanding of HIPAA and confidentiality. Strong communication and listening skills. Ability to work independently to meet client needs. Travel required to client homes or community settings. Sitting, standing, bending required. Ability to lift 25 pounds regularly.
