Hospice RN Case Manager - In-Home Care & Equity Options

Wythe Hospice, part of LHC Group, is seeking a Registered Nurse Case Manager to deliver in-home hospice care and oversee Plan of Care for patients. You will evaluate needs, initiate care plans, and coordinate with the IDG and attending physicians to support physical, emotional, and spiritual well-being. The role requires Virginia RN licensure, a valid driver’s license, and willingness to travel within the service area.
