Work Schedule:

Primary hours are Monday, Tuesday, and Thursday from 8:00 a.m. - 5:00 p.m., with some evening work until 7:00 p.m. as needed.

Primary Accountability

Functions collaboratively with site Providers, Health Center Manager, and the Director of Special Programs as a member of the Student Health Center’s team. Responsible for helping patients and their families to navigate and access community services, other resources and adopt healthy behaviors. The Community Health Worker builds individual and community capacity by increasing health knowledge and self-sufficiency through a range of activities such as outreach, community education, informal counseling, social support and advocacy. Represents InterCare at the Student Health Center along with performing the front office functions of the clinic.

Description of Primary Duties & Responsibilities

1) Functions as Front Office Specialist for the Student Health Center as well as overall responsibility for the Center.

• a) Greets and assists patients in navigation of their visit.
• b) Registers new and existing patients for their scheduled appointment.
• c) Accurately enters patient demographic and insurance information into the electronic practice management system.
• d) Accurately identifies patient visit type as confidential following InterCare’s policy on confidentiality.
• e) Assesses the need for insurance enrollment, sliding fee eligibility, and state funded assistance programs.
• f) Schedules follow-up and walk-in appointments as requested.
• g) Pulls MCIR as needed for patient visit.
• h) Coordinates daily flow and activities within and outside the clinic including facility access.
• i) Orders supplies in a frugal financially responsible way (mindful of the monthly budget).
• j) Responsible for retrieving mail from the main school site, other InterCare locations, and post office as needed.
• k) Monitors and assesses all facility needs for the clinic.
• l) Maintains order and cleanliness within the Health Center throughout the day.
• m) Coordinates needed repairs to maintain a safe and secure environment.
• n) Maintains and updates all state and regulatory signage to comply with various legal requirements.
• o) Maintains petty cash processing and balancing.
• p) Performs various clerical duties including scanning and faxing of documents and identification cards.
• q) Creates letters, including student notes and communication as needed.

2) Responsible for establishing trusting relationships with patients and their families while providing general support and encouragement.

• a) Provide ongoing follow up, basic motivational interviewing and goal setting with patients/families.
• b) Conduct intake interviews with patients, including enrolling and/or referring patients into MI Bridges, Sliding Fee Program, Healthy Michigan Plan and other programs.
• c) Assists patients with completing assistance program applications and registration forms.
• d) Conduct eligibility determination, enrollment and follow-up with uninsured patients.

3) Works closely with an integrated care team to help ensure patients have comprehensive and coordinated care.

• a) Responsible for providing consistent communication to provider care team to evaluate patient/family status, ensuring that provided information, and reports clearly describe progress.
• b) Reduce stigma and other barriers to initiating or continuing health care by providing culturally/linguistically humble care, reliable information to both community members and health care providers.
• c) Help patients set personal goals, and attend appointments.
• d) Provide referrals for services to community agencies as appropriate.
• e) Follow-up with patients via phone callsand office visits..
• f) Assist patient in understanding care plans and instructions.
• g) Coach patients in effective management of their chronic health conditions and self-care.
• h) Act as a patient advocate and liaison between the patient/family and community service agencies (i.e. schools, Department Human Services, hospitals, support groups, etc.).

4) Serves as Community Outreach coordinator for all school related meetings and events.

• a) Assists with the implementation and ongoing compliance with all organizational, administrative, and school-based program policies, procedures, and regulations.
• b) Prepares for and provides outreach at school sporting events, teacher meetings, parent/teacher conferences, and other school events.
• c) Assists and supports all committees and meetings related to the School Health Center.
• d) Serves as primary liaison with the school district promoting awareness of services.
• e) Assists with data collection and reporting to meet school based program requirements.

1. Maintains BCLS Certification is required.

7. Maintains BLS requirement. BLS is a mandatory by InterCare and our FTCA agreement with the Federal Government. It must be completed within 90 days of hire.

Professional and Technical Knowledge

1. Possesses a high school diploma or equivalent.
2. Successful completion of 126 hours of the Community Health Worker Michigan-specific training, 4 hours of MI Bridges Medicaid enrollment training certification training, and completion of the Certified Application Counselor (CAC) curriculum.

Technical Skills

1. Basic to intermediate level skills in Microsoft Office Suite.
2. Ability to present educational information to patients one-on-one or in group settings.
3. Ability to create letters, forms, documents, presentations.
4. Knowledge of medical terminology preferred.

Communication Skills

1. Possesses a professional level of written and verbal communication skills.
2. Ability to communicate complex concept in a clear effective manner.
3. Possesses excellent cross cultural communication skills and the ability to communicate to staff members at all levels.
4. Fluent in Spanish and English, reading, writing, and speaking skills.

Physical Demands

Job duties are performed in the typical office environment of the organization. The environmental factors and/or physical requirements of this position include the following:

1. Ability to stand, walk, stoop, kneel, crouch, frequently sit, and lift up to 25 lbs.
2. Requires good hand-eye coordination, reach with arms & hands, and finger dexterity, including ability to grasp, and manipulate (using keyboard, office equipment).
3. Visual acuity to use keyboard, operate office equipment, and read printed material and regularly required to talk and hear.
4. Must be able to drive a vehicle.
