To support a tech-enabled medication management service, the part-time PRN Maryland Licensed Clinical Pharmacist will provide comprehensive medication management and patient counseling remotely, focusing on optimizing patient outcomes in collaboration with healthcare providers. Key responsibilities Conduct comprehensive medication reviews and maintain accurate patient records Identify and resolve medication-related problems, providing evidence-based recommendations Collaborate with healthcare professionals to ensure coordinated patient care and participate in quality improvement initiatives Required qualifications Doctor of Pharmacy (Pharm.D.) from an accredited institution with 3 years of clinical experience Current active pharmacist license in Maryland; licensure in multiple states is preferred National Provider Identifier (NPI) required Board of Pharmaceutical Specialty Certification or ASHP Residency completion preferred Spanish fluency preferred at a healthcare professional level
