Licensed Practical Nurse (LPN) – Home Care

Location: Oʻahu, Hawaiʻi
Job Type: Part-Time & Per Diem
Compensation: Starting at $26.00/hr

Position Summary:
We are currently seeking skilled and caring Licensed Practical Nurses (LPNs) to join our Home Care team on Oʻahu. This role is ideal for nurses looking for a flexible schedule with the opportunity to work one-on-one with clients in their homes. Part-Time and Per Diem positions are available.

Responsibilities:

• Provide hands-on nursing care in accordance with individualized care plans
• Conduct home visits to monitor client health and progress
• Administer prescribed medications and treatments
• Document observations and communicate any changes in client condition
• Collaborate with RNs, physicians, and other care team members
• Offer patient and caregiver education on basic health maintenance
• Ensure adherence to all safety and clinical guidelines

Qualifications:

• Active and unencumbered LPN license in the State of Hawaiʻi
• Minimum of one (1) year of nursing experience (home care experience preferred)
• Current CPR (BLS) certification
• Strong clinical and organizational skills
• Compassionate, professional, and reliable
• Valid driver’s license, reliable transportation, and proof of auto insurance
