The LPN / RN plays a critical role in delivering high-quality nursing care within a clinic healthcare setting. This position focuses on providing direct patient care, including telehealth and clinic setting, ensuring patients' comfort and safety. The LPN / RN will also be responsible for educating patients and their families about health conditions and care plans, promoting better health outcomes. Collaboration with Nurse Practitioners and other healthcare professionals is essential to coordinate care and implement treatment plans effectively. Ultimately, the LPN / RN contributes to maintaining a supportive and healing environment that enhances the quality of life of our communities.

Minimum Qualifications:

• Valid and current Kentucky Nursing License as a Licensed Practical Nurse (LPN) or Registered Nurse (RN)
• Proficiency in reading, writing, and speaking English to communicate effectively with patients and healthcare team members.
• Demonstrated experience in patient care and patient education.
• Ability to perform venipuncture safely and accurately.

Preferred Qualifications:

• Previous collaboration experience with Nurse Practitioners or advanced practice providers.
• Familiarity with electronic health record (EHR) systems.
• CPR and Basic Life Support (BLS) certification.
• Experience in telehealth

Responsibilities:

• Provide comprehensive patient care including monitoring vital signs, administering medications.
• Perform wound care management, including assessment, cleaning, dressing changes, and documentation.
• Conduct venipuncture procedures safely and efficiently for laboratory testing and other medical needs.
• Educate patients and their families on health maintenance, disease prevention, and treatment plans in clear and understandable English.
• Collaborate closely with Nurse Practitioners and interdisciplinary teams to develop and implement individualized care plans.
• Maintain accurate and detailed patient records in compliance with regulatory standards and facility policies.
• Ensure compliance with all nursing licenses and facility regulations to uphold the highest standards of care.

The required skills such as patient assessment, patient education, and venipuncture are applied daily to ensure patients receive comprehensive and compassionate care. Proficiency in English enables clear communication with patients, families, and healthcare teams, which is vital for effective education and coordination. The nursing license confirms the candidate's ability to perform clinical tasks safely and within legal scope. Preferred skills like collaboration with Nurse Practitioners and familiarity with EHR systems enhance interdisciplinary teamwork and streamline documentation, contributing to efficient and high-quality patient care.
