Job Summary:

We are seeking a compassionate and skilled LPN Licensed Practical Nurse to join our team at Dignity HCC in Forest City, PA. As an LPN at Dignity HCC, you will play a vital role in providing high-quality, personalized care to our residents. About the Role:

As an LPN at Dignity HCC, you will be responsible for:

Providing direct patient care, including administering medications, performing vital signs, and conducting wound care
Assisting with daily living activities, such as bathing, dressing, and grooming
Maintaining accurate and detailed records of patient care and medication administration
Collaborating with the healthcare team to develop and implement individualized care plans
Providing emotional support and education to patients and their families
Participating in quality improvement initiatives to ensure optimal patient outcomes

Requirements:

Current LPN licensure in the state of Pennsylvania
