Pay and Hours Hourly Pay Range: $36.00 - $55.80. Position: Registered Nurse Home Visits – Family Connects – Full Time. Hours: 32 per week, Monday‑Thursday 8:00am–4:30pm. Location: Chicago, IL. Travel: provides home visiting to all Swedish Hospital newborns and families within Chicago. Job Description The Family Connects model, developed within the Durham Family Initiative and funded by the Duke Endowment, aims to reduce community rates of child maltreatment through evidence‑based assessment of child and family strengths and needs. As a Family Connect Community Home Visiting nurse, you will provide home visiting services to families and newborns born at Swedish Hospital that reside within the city of Chicago, supporting and assessing their physical and psychological well‑being, and facilitating referrals to community resources. Responsibilities Visit families during the immediate postpartum period in the patient’s home or via virtual appointment as preferred. Follow the Family Connects home‑visiting protocol, keeping required records of client observations and activities. Document information for family assessments and program evaluation, recording data in real time via the computerized record system. Visit inpatient settings as needed to schedule home visits for new mothers. Assess family needs across 12 domains of physical and psychological well‑being, including post‑partum depression screening. Assist with referrals to community agencies to support families. Facilitate and teach community‑based classes at the community center. Apply effective time‑management, organization, and multitasking skills. Maintain 10 open visit slots and conduct and document at least 7 home visits per week. Respond to family needs through flexible scheduling, including evenings or weekends. Participate in weekly team meetings, clinical supervision, staff meetings, and other required gatherings. Support and encourage development of other staff and the Family Connects Program. Qualifications B.S., B.A. (other field), or A.S. degree in nursing with a current RN license, or RN license with at least one year of relevant nursing experience. American Heart Association CPR certification. Valid driver’s license and a vehicle for home visits. Experience as a registered nurse in a hospital or clinic, preferably OB. Computer skills: Word, Outlook, Excel and other programs; comfortable using a laptop during in‑home assessments. Bilingual in Spanish and English preferred; ability to use translation services. Experience in a clinical setting and dealing with customers preferred.Excellent organizational skills and ability to prioritize tasks independently with minimal supervision. Ability to work with culturally diverse populations. Preferred: 1+ year in child/maternal health, social work, or related field; or Certified Lactation Educator or Consultant.
