Summary We are seeking a licensed Physical Therapist (PT) to join our interdisciplinary pain management team. This role is responsible for providing comprehensive physical therapy services to patients with chronic pain and functional impairments in an outpatient clinical setting. The Physical Therapist collaborates closely with providers, medical assistants, and behavioral health staff to design and implement individualized treatment plans focused on improving mobility, reducing pain, and restoring function. Candidates with experience in pain management, orthopedic rehabilitation, functional medicine, or integrated medical practices are strongly encouraged to apply. Responsibilities Perform comprehensive physical therapy evaluations and ongoing reassessments of patient functional status Develop and implement individualized treatment plans based on clinical findings and physician referrals Provide therapeutic exercises, manual therapy, and physical interventions to improve mobility and function Administer treatments including soft tissue mobilization, traction, ultrasound, and other approved modalities Instruct and motivate patients in therapeutic exercises and activities of daily living Educate patients and families on home exercise programs and injury prevention strategies Monitor patient response to treatment and adjust care plans as clinically appropriate Document all evaluations, treatments, and progress notes in the electronic medical record in accordance with clinical and regulatory standards Communicate assessment findings and progress updates to referring physicians and clinical staff Coordinate care with other members of the interdisciplinary healthcare team Provide clinical supervision and guidance to Physical Therapy Assistants when applicable Ensure all patient care is delivered safely, timely, and in alignment with documented treatment goals Required Qualifications Graduate of an accredited Physical Therapy program Active Physical Therapist license in the State of Illinois Current Physical Therapist certification Minimum of 2 years of clinical experience preferred Proficiency with EMR documentation Strong communication and patient education skills Ability to work effectively within an interdisciplinary medical team Preferred Qualifications Experience in pain management, orthopedics, or outpatient rehabilitation Experience working in an integrated medical practice setting Manual therapy certification or advanced musculoskeletal training Experience supervising Physical Therapy Assistants Familiarity with chronic pain patient populations Behavioral & Professional Expectations Demonstrates professionalism, empathy, and patient-centered care Maintains a positive and collaborative team-oriented attitude Communicates clearly with patients, families, and clinical staff Adheres to clinic policies, safety protocols, and confidentiality standards Participates in ongoing professional development and quality improvement initiatives Contributes ideas that improve patient care, clinic efficiency, and operational performance Physical & Mental Demands Ability to lift and assist patients safely Manual dexterity for therapeutic interventions Ability to remain on feet for extended periods and perform desk-based documentation Capacity to work with patients experiencing pain, stress, or emotional distress Demonstrates patience, resilience, and professional judgment #J-18808-Ljbffr
