Queens Community House is seeking a compassionate, organized, and experienced Case Manager to join our Older Adult Services team. Case Management services play a vital role in assisting older adults with functional impairments by connecting them to the services, benefits, and resources they need to remain safely in their homes while maintaining their independence and quality of life.

The Case Manager will manage a caseload of approximately 70+ older adults residing in Community Districts 4 and 6, including Elmhurst, Corona, Forest Hills, and Rego Park. Through comprehensive assessments, advocacy, care planning, and collaboration with families and community partners, the Case Manager will empower clients to achieve the highest level of independence possible.

Essential Responsibilities

Client Assessment & Case Management

• Screen prospective clients through the intake process to determine eligibility for case management services.
• Conduct comprehensive in-home assessments and periodic reassessments to evaluate client needs and continued eligibility.
• Assess clients' physical, emotional, financial, social, and family support needs.
• Develop individualized, culturally responsive care plans that maximize available community resources.
• Build trusting relationships with clients and caregivers through a collaborative, strengths-based approach.
• Monitor client progress and update care plans as needed.

Service Coordination & Advocacy

• Provide accurate information regarding available benefits, entitlements, and community resources.
• Coordinate referrals and arrange supportive services to meet client needs.
• Advocate on behalf of clients with healthcare providers, government agencies, and community organizations.
• Engage family members and informal support systems to strengthen ongoing care and support.

Documentation & Compliance

• Maintain accurate, timely, and complete case records in accordance with agency and funding requirements.
• Prepare required reports and documentation.
• Ensure confidentiality of client information and compliance with all applicable regulations.

Supplemental Services & Resource Navigation

• Assist clients with accessing Supplemental Services Funds by identifying eligible needs and coordinating purchases and deliveries.
• Connect clients and families with additional Queens Community House programs and external community resources through cross-department referrals.

Outreach & Community Engagement

• Participate in outreach initiatives to identify and engage older adults who may benefit from case management services.
• Represent Queens Community House professionally within the community and with partner organizations.

Professional Development & Team Collaboration

• Participate in New York City Aging and other required internal and external trainings.
• Attend staff meetings, supervision sessions, annual evaluations, and QCH Staff Development Day.
• Collaborate effectively with colleagues across departments to ensure high-quality client services.
• Perform other duties as assigned.

Qualifications

Required

• Bachelor of Social Work (BSW) or related bachelor's degree.
• Minimum of two (2) years of experience working with older adults or the gerontological population.
• Strong understanding of aging services, benefits, and community resources.
• Demonstrated cultural competence and sensitivity to the diverse backgrounds, needs, and preferences of older adults.
• Knowledge of effective communication strategies for individuals with visual and/or hearing impairments.
• Excellent interpersonal, customer service, and relationship-building skills.
• Strong organizational skills with exceptional attention to detail.
• Ability to manage multiple priorities while maintaining accurate documentation.
• Critical thinking and problem-solving skills with the ability to navigate complex client situations.
• Proficiency with Microsoft Office and other computer applications.
• Ability to maintain confidentiality and exercise sound professional judgment.

Preferred

• Bilingual in English and Spanish.
• Experience using the Senior Tracking, Analysis, and Reporting System (STARS) database.
• Experience working within community-based aging services or case management programs.
