Student and Family Care Navigator, District Wide Lincoln County Schools – Lincoln County Schools-ISC, Lincolnton, North Carolina. Position effective August 6, 2026. Job Details Job ID: 5719919 Application Deadline: Open until filled Posted: May 18, 2026 Terms of Contract: 10 months, full-time, benefit eligible (1-year contract with option for renewal up to 5 years pending grant approval). Reports To: Director of Student Support Services Salary: State Schedule plus local 9% supplement. FLSA: Exempt Responsibilities Manage a targeted, district-wide caseload of students identified as at highest risk for substance use and related challenges. Serve as a family navigator, working directly with parents to address social determinants of health and remove barriers to care. Provide direct connections and supports for families to the Lincoln County Health Department, local substance-abuse resources, and other community resources. Collaborate with community agencies, non-profit partners, and public-health stakeholders to maintain a current resource network and coordinate wrap-around support. Offer mentoring and follow-up support for students to ensure continuity of care. Collaborate with school administrators, counselors, and social workers to identify appropriate students for referral to this caseload. Maintain accurate, confidential documentation in compliance with FERPA and state/federal requirements. Collect, maintain, and submit accurate data for grant compliance and reporting. Complete required professional development, including NCFADS courses and state-endorsed continuing education. Submit required district, health-department, and state grant reports in a timely and accurate manner. Uphold all requirements of the NC Parents' Bill of Rights in all interactions. Perform occasional heavy work involving lifting up to 100 pounds and frequent lifting up to 50 pounds, as required. Other duties as assigned. Qualifications Required: Current North Carolina license as School Social Worker, School Counselor, or Licensed Clinical Social Worker (LCSW). Must have experience working with at-risk youth, substance-use prevention, or community health navigation.
