We're seeking a dedicated Care Manager to join our Health Home Serving Children (HHSC) program—supporting youth with complex needs to achieve wellness, stability, and success.About the Role:As an HHSC Care Manager, you'll serve as the primary point of contact for children with chronic conditions or behavioral health challenges and their families. You'll coordinate care across providers, schools, and support systems to ensure a comprehensive, child-centered plan of care is in place and followed.Your work will span Brooklyn and Queens, meeting children and families where they are—at home, in the community, or virtually.Key Responsibilities:Conduct comprehensive assessments and develop individualized care plansCoordinate medical, behavioral health, educational, and social servicesServe as a liaison between families, providers, schools, and support systemsTrack and document progress toward health, social needs, and wellness goalsCoordinate transitional care from Emergency Room/Inpatient Admissions to ensure safe discharge to the communityAdvocate for children's needs while empowering familiesAttend regular supervisory and case conference sessions, trainings and other meetings, as assigned.Who You Are:Passionate about child welfare, behavioral health, and equity in healthcareSkilled in communication, cultural sensitivity, and problem-solvingOrganized, proactive, and able to work independently and collaborativelyComfortable navigating Brooklyn/Queens neighborhoods for field visitsQualifications:Bachelor's degree in social work, psychology, sociology, education, nursing, public health, or a related fieldAt least 2 years of relevant experience working with children/familiesKnowledge of Medicaid, Public Assistance, SNAP, and other and community-based services a plusValid NYS Driver's License or reliable transportation for community visitsWhy Join Us?Supportive, mission-driven team cultureOngoing professional development and trainingCompetitive salary and full benefits (health, dental, PTO, 401k)Opportunity to grow within a dynamic, expanding care coordination program
