Your Role

Provide virtual clinical care including assessments, diagnosis, treatment planning, medication management, preventive care, chronic care support, patient education, and follow-up.

More importantly, you will help guide patients throughout their care journey—connecting virtual and in-person care rather than treating each encounter as an isolated visit.

Who We're Looking For

We are seeking licensed Physicians, NPs, and PAs who:

• Have experience in primary care, family medicine, internal medicine, urgent care, or related specialties
• Believe in building long-term, patient-centered relationships
• Are comfortable delivering care virtually
• Want to participate in innovative and evolving care models
• Are comfortable using digital and AI-enabled clinical tools
• Believe healthcare should be continuous, coordinated, accessible, and centered around the patient
