POSITION SNAPSHOT

Location: Sharon Medical Group Behavioral Health 63 Pitt Street Sharon PA 16146

Schedule: Monday - Friday 8:00am - 4:30pm No Weekends / No Holidays

Employment Status: FULL-TIME 

Compensation: Starting pay: $20 per hour

WHY THIS ROLE

Join a collaborative healthcare team focused on safe, patient-centered care. PHN's Licensed Practical Nurse provides practical nursing care within the LPN scope of practice, supporting patient visits, medication and treatment needs, care coordination, documentation, and communication across the care team.

WHAT YOU'LL DO

· Provide practical nursing care consistent with the plan of care, provider orders, state scope-of-practice requirements, competency, and PHN policy.

· Room patients, obtain and document appropriate information, and prepare patients for exams or procedures.

· Collect focused patient data, observe changes in condition, and communicate significant findings to the appropriate care team member.

· Administer medications and perform therapeutic treatments when authorized by law, policy, valid order, and demonstrated competency.

· Collect specimens and perform point-of-care testing and other approved clinical tasks.

· Document care, patient responses, communications, and follow-up accurately in the electronic medical record.

· Reinforce patient and family education and assist with care coordination.

· Perform injections and venipuncture when included in approved clinical privileges.

WHAT WE'RE LOOKING FOR

· Graduate of an approved practical nursing program.

· Current, valid LPN license in the state of practice; Pennsylvania and/or Ohio licensure required based on assigned location and responsibilities.

· Current CPR/BLS.

· Demonstrated competency in practical nursing skills, medication administration, infection prevention, documentation, patient communication, and teamwork.

· Ability to recognize changes in patient condition and practice within legal scope and demonstrated competency.

· Strong interpersonal, verbal, written, organizational, and multitasking skills.

· Ability to use the electronic medical record and related clinical systems effectively.

COMPENSATION

Starting pay: $20 per hour. PHN recognizes experience, education, credentials, and site location when determining wage, as applicable.
