Job OpportunityWe are seeking an experienced healthcare professional to provide skilled home care services. This role requires strong clinical skills and the ability to manage a range of patient diagnoses in a home setting.Shift DetailsShift: Day 5x8-Hour (08:00 - 17:00)Contract Duration: Various contract lengths availableWeekend Coverage: Minimum 1-2 weekends per contractJob ResponsibilitiesConduct home skilled visits, averaging 6-8 per day.Serve patients with diagnoses including but not limited to: Med/surgCardiovascular conditionsOrthopedic conditions (hips, knees, shoulders)Post-surgical recoveryRespiratory conditions (COPD, CHF, pneumonia)Neurological conditions (CVA, Parkinson's, trauma)Amputations (AKA, BKA)Activities of Daily Living (ADLs)Maintain accurate and timely documentation using designated systems.Collaborate with interdisciplinary teams including physical therapy and social services.QualificationsMinimum of 1 year of home health experience required.OASIS training preferred.Current BLS certification required.Valid driver's license and reliable vehicle for home visits.Must be fully vaccinated against COVID-19, with exemptions considered.Annual flu vaccination required or valid declination with mandatory mask-wearing while at work.Skills RequiredProficient in patient assessments and care planning.Experience with adaptive equipment and rehabilitation techniques.Skilled in wound management and trauma care.Knowledge in cognitive and functional assessments.Ability to develop and implement home exercise programs.Strong communication and patient education skills.LocationThis position is available in various locations within the state.Application ProcessIf you meet the qualifications and are interested in this opportunity, please apply with your updated resume and relevant certifications.
