As a member of the Health Support Center (HSC) team, you’ll support those in our facilities who are interfacing and providing care to our patients and community members to positively impact our mission of making communities healthier. Responsibilities Assist patients with performing activities of daily living. Provide or assist with providing direct patient care, diagnostic procedures and therapeutic interventions. Observe and record patient behavior and activities. Report any changes in physical, mental or emotional condition to nursing staff and suggest modifications to the treatment plan. Provide crisis intervention with incoming calls and on the unit to maintain safety. Qualifications We are looking for a compassionate nursing assistant with a deep understanding of long‑term care, with a focus on patient enrichment.
