Nurse Practitioner

Duration: 6 contracts

Position Summary:
Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidence-based care to a panel of patients. The clinical care team will work with the provider daily to drive better outcomes for the entire patient panel. This includes accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. This role reports to the Practice Manager.

Key Notes:

• Application for DEA registration may be required during onboarding unless otherwise noted.
• Position may require working every other weekend with possible holiday rotations unless otherwise specified.

Responsibilities:

• Provide primary and family evidence-based care, including accurate assessment, diagnosis, treatment, management of health conditions, health counseling, and disposition planning.
• Perform administrative duties including opening/closing the clinic, inventory management, patient follow-up, insurance verification, payment collection, and maintaining a clean environment.
• Manage patient appointments and perform routine examinations to ensure positive patient health outcomes.
• Educate patients on healthy lifestyle choices and wellness.
• Complete diagnostic tests to diagnose and treat illnesses or injuries.
• Coordinate with healthcare professionals to maintain a comprehensive care plan.
• Support patient engagement through health screenings, behavioral health and wellness, and chronic condition management.

Required Experience & Skills:

• Minimum 1 year of recent Primary Care experience as a Nurse Practitioner in family medicine, primary care, or urgent care.
• Working knowledge of quality measurement and chronic disease management.
• Self-motivated with strong problem-solving and patient advocacy skills.
• Strong verbal, written, and electronic communication skills.
• Excellent organizational and multitasking abilities.
• Ability to work autonomously and without direct supervision.
• Comfortable working remotely within a clinical care team environment.
• Proficient with healthcare technology and information management systems.
• Willingness to obtain multi-state licensure.

Education Requirements:

• Master's Degree from a Family Nurse Practitioner program.
• Current National Board Certification.
• Active state license to practice in the Advanced Practice Nurse role.

Additional Requirements:

• Verifiable High School Diploma or GED required.
• Candidate questionnaire must be included with the resume.
• Prior client experience not required.
