Overview Altus Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager (RNCM) to join our dedicated nursing team and provide exceptional end‑of‑life care throughout the community. Schedule Per Diem, Monday – Friday, 8 am – 4:30 pm. Territory Temple and Waco. Role In this rewarding role you will deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. Collaborate with the Clinical Director to assess eligibility and suitability of clients for specialized hospice services. Serve as a source of comfort and support for patients, ensuring their comfort and quality of life remain paramount. Deliver nursing care rooted in proven principles and techniques, helping ease pain and bring solace to those under our care. Educate and advocate for patients and families, encouraging active participation in personalized care plans. Participate in call duties as part of the team’s commitment to being there when it matters most. Qualifications Current, unencumbered license as a Registered Nurse. Minimum one year of experience as an RN in a medical‑surgical/acute‑care setting; hospice experience a plus. Proficiency in computer use and familiarity with EMR systems. Current CPR certification if required by state. Valid driver’s license, vehicle insurance, and reliable transportation. Compensation $36.00 – $42.00 per hour (average pay range).
