Outpatient Physical Therapist

This is a 16-week contract position.

This position operates on a 5x8.0 shift.

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans according to the patients' needs, goals, and physician direction. The therapist is responsible for delegating and overseeing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate an effective return to community activities. This role also includes educating patients, families, students, and coworkers while performing professional duties associated with patient care and the development of therapy services.

Responsibilities:

• Completes comprehensive evaluations of assigned patients and develops appropriate treatment plans and goals.
• Documents medical records accurately and in a timely manner, complying with relevant guidelines and the clinic's policies.
• Implements care under signed physician orders and insurance approvals as necessary.
• Participates in student programs, fostering a positive learning environment with proper supervision.
• Maintains a professional development plan and actively pursues continuing education and professional involvement.
• Adheres to the discipline-specific State Practice Act and maintains professional licensure in good standing.
• Understands organizational performance improvement processes and contributes to quality service enhancements.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited school.
• Two years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state license as required for practice.
• Previous successful assignment with the agency is highly desired.
