Provides direct job development services and employment support services to assist to obtain and maintain community-based employment. Works as a team member of a clinical team to assess member needs and wants, participates in the development of individual service plans, acts as a member advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Key Responsibilities Develop individualized employment plans in collaboration with the members, focusing on skill development, job readiness, and job placement. Provide vocational counseling, coaching, and support to members in job search strategies, resume writing, interview preparation, and workplace behavior. Establish partnerships with employers, vocational training programs, and community organizations to create job opportunities for clients. Collaborate closely with other members of the ACT team, including Rehabilitation Specialists, BHMP, and clinical team to coordinate comprehensive care and support for the members. Meets with the members in the community in accordance with established policies and procedures and clinical protocols to provide supportive counseling skill development and other support services as needed. Completes all clinical documentation in timely manner in accordance with policies, procedures, and clinical protocols, and meets/exceeds member engagement goals as defined by Copa Health. Qualifications High school diploma or high school equivalency diploma and four (4) years of full-time direct care experience in behavioral health or Bachelor’s Degree in a related field OR a bachelor’s degree in a non-related field with one (1) year experience preferred. Must have 1 year of training or experience in vocational rehabilitation and support. Prior experience may be job development, job coaching, or supported employment. Must have a Level One Fingerprint Clearance Card by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire. TB Test clearance required at the time of hire. Must have reliable transportation, valid AZ Driver’s License with state minimum insurance coverage and ability to meet and maintain Copa Health’s acceptable driving requirements.
