Registered Dietitian - Center Line

Summary Under the general direction of the Clinical Manager, the Registered Dietitian (RD) implements the Nutrition Care Process (NCP) by completing comprehensive nutrition assessments, determining nutrition diagnoses, developing and implementing evidence‑based nutrition interventions, and evaluating participant progress. The RD performs assessments for malnutrition and morbid obesity, evaluates food insecurity, motivates behavior change, and develops individualized goals for participants with complex medical conditions. Specific Duties and Functions Participant Care Nutrition Assessment: Conducts a comprehensive nutrition assessment at enrollment and semi‑annually according to CMS guidelines. Utilizes screening tools, medical history, laboratory data, medications, anthropometric measurements, and diet history. Performs nutrition focused physical exams when indicated and evaluates psychosocial, functional, and behavioral factors related to food access, selection, preparation, physical activity, and understanding of health conditions. Assesses participant and family knowledge and readiness for positive outcomes and collaborates with IDT members when indicated. Nutrition Diagnosis: Identifies patterns and possible causes for nutrition problems, documents diagnostic statements in PES format, and supports evidence within documentation. Nutrition Intervention: Formulates individualized interventions focused on the etiology of the problem. Assesses educational needs and provides tailored nutrition education and counseling based on individual nutrient requirements, medical, economic, and social situations. Coordinates nutrition plans with IDT teams and other healthcare professionals when indicated. Nutrition Monitoring and Evaluation: Uses monitoring and evaluation techniques that consider participants’ nutritional needs as well as cultural, religious, and ethnic concerns. Evaluates intervention effectiveness, measures progress toward outcomes, and provides timely follow‑up for high‑risk participants based on departmental criteria and clinical judgment. Documentation, Coordination of Care, and Compliance: Completes relevant, accurate, and timely documentation in the participant’s electronic medical record in accordance with CMS, PACE SEMI, and departmental standards. Participates in Care Plan meetings as a required IDT member and communicates care plans effectively with interdisciplinary team members and other healthcare providers as warranted. Knowledge, Skills and Abilities Professional knowledge from a Bachelor’s Degree in dietetics, nutrition, or a related field, completion of an ACEND‑accredited dietetics program, and successful registration exam. One year of experience working with aging adults preferred. Ability to maintain interpersonal and interdepartmental relationships, apply adult learning principles, and organize educational activities for participants and caregivers. Strong analytical skills for problem solving and developing viable interventions. Meets annual competency requirements, participates in quality improvement projects, and attends professional trainings and required screenings. Working Conditions Variable conditions with potential exposure to communicable disease in the participant environment. Driving required within the PACE Southeast Michigan catchment area and potential exposure to extreme temperatures.
