Physical Therapist PT

Position Summary Responsible for evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referrals, state and national standards, and department policies. Shift and Schedule Day Shift, Monday-Friday 8:00–5:00 pm. Includes rotating weekend schedule: 1–4‑hour Sunday shift every 6–7 weeks. Core Functions Evaluates patient’s functional abilities and limitations; determines if intervention is needed; establishes a plan of care and treatment goals; completes comprehensive evaluation within the department‑set time frame; identifies and prioritizes key limiting factors; establishes functional, measurable, patient‑related goals that reflect the key limiting factors; collaborates with patient and family/caregiver when setting goals; initiates discharge planning; recommends additions to or modifications of referring orders. Implements individualized treatment plan considering the patient’s physical, social, spiritual, cultural, educational and age‑specific needs; provides skilled treatment safely; assesses ongoing effectiveness of treatment and modifies plan of care as indicated; involves patient and family/caregiver in treatment and decision‑making; provides ongoing education to physician, nurse, patient, family/caregiver, and interdisciplinary team. Documents according to professional practice guidelines set by regulatory agencies; documents legibly; completes documentation within established time frames; documents evidence‑based and individualized treatment techniques; records skilled intervention and comprehensive assessment of patient progress or lack thereof; reassesses treatment goals according to department policies. Communicates pertinent patient information to ensure optimal follow‑through of care; relays evaluation/treatment results and patient progress to physicians, nurses, patients, family/caregivers, and interdisciplinary team; contacts physicians for routine or problem‑solving issues to ensure optimal patient outcomes; defends treatment rationale with standard practices and current literature. Exhibits adaptability in assignments related to caseload, scheduling, and staffing shifts; takes initiative to assist in other patient care areas and accepts responsibility for all patient care needs. Demonstrates appropriate supervision and delegation of personnel to achieve safe delivery of quality, cost‑effective patient care; supervises PTA, ATC, therapy technicians, and students in compliance with department policies and regulatory agencies; participates in training and orientation of new personnel; ensures competency of personnel before delegation of tasks. Participates in department operational activities: attends required meetings, follows up to obtain pertinent information, contributes to departmental/facility process improvement, and meets productivity standards set by the department. Manages interactions with all staff levels in various departments, physicians, patients, families, and external contacts such as employees of other health care institutions, community providers, and agencies concerning patient health care; also interacts with physician offices to report, request, or clarify information. Minimum Qualifications Graduate from an APTA accredited school of Physical Therapy or equivalent. Current Physical Therapy license in the State of practice. BLS certification required. Demonstrated ability to become proficient with computer software applications commonly used in an automated patient care environment, including patient care equipment, evaluation tools, therapeutic modalities, copier, multi‑line telephone, fax, computer, pager, and miscellaneous office equipment. For employees at Banner Home Care/Hospice or Boswell Skilled Nursing Facility, possess and maintain an Arizona Fingerprint Clearance Card at the time of hire and for the duration of employment. Preferred Qualifications Additional related education and/or experience preferred.
