Job Description:

Seeking a Physical Therapist to provide home health services in the Valley City, ND area, covering a 60-mile radius. The role involves treating 5-7 patients per day, focused on home health and hospice patients.

Responsibilities:

• Provide physical therapy to patients with various diagnoses including surgeries and falls.
• Document patient progress using the HomeCare/HomeBase EMR system.

Requirements:

• BLS Certification and North Dakota Physical Therapy License.
• Minimum of 1 year of physical therapy experience, with previous home health experience required.
• Valid Driver's License and Auto Insurance.
