Key Responsibilities

Evaluate and treat patients with speech, language, voice, and swallowing disorders in a skilled nursing facility setting
Develop and implement individualized treatment plans based on patient assessments and clinical goals
Monitor and document patient progress, adjusting treatment plans as clinically appropriate
Collaborate with interdisciplinary care teams to support comprehensive patient outcomes
Maintain accurate and timely clinical documentation in accordance with facility and regulatory requirements

Required Qualifications

Active Speech Language Pathologist (SLP) license in the state of Colorado
Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA preferred or eligibility required per facility standards
Experience working in a skilled nursing facility or long-term care environment
Strong clinical documentation and communication skills

Preferred Experience / Skills

Prior experience with dysphagia evaluation and treatment, including modified barium swallow studies
Familiarity with MDS/PPS documentation processes in a SNF setting
Experience with geriatric patient populations
