Physical Therapist - Rehabilitation

Schedule
Shift: 8 hours

Assignment Details
Contract Length: 14 weeks
Guaranteed Hours: 40 per week

Requirements
- Current State License as a Physical Therapist - Required
- Proof of current CPR certification - Required
- Current driver's license; must have reliable transportation and insurance - Required
- 1-2 years experience as a Physical Therapist in an acute care or rehabilitation setting - Required
- Previous home care experience - Preferred
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program - Required

Roles & Responsibilities
- Perform clinical assessments to identify patient needs and appropriateness for therapy.
- Document and review patient care plans, progress notes, and other clinical records on an ongoing basis.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders based on evaluations and medical referrals.
- Instruct patients and families on transfer techniques and the use of therapeutic equipment safely.
- Provide education regarding therapeutic procedures tailored to patient age, condition, culture, and learning readiness.
- Collaborate with an interdisciplinary team and consult with physicians as necessary.
