About This Role As a Home Health Registered Nurse at Cleveland Clinic, you will provide high-quality nursing care to patients in their homes, focusing on assessments, complex treatments, and education to prevent hospital readmissions. Highlights Pay: $69,680 - $111,488 annually Location: Independence, OH Facility: Cleveland Clinic Fairview Hospital Schedule: Full-time, Day shift What You’ll Do Provide direct nursing care to patients in their homes. Perform accurate and complete admission assessments and reassessments. Assess patients for safety risks, abuse, and neglect. Serve as a primary coordinator of all disciplines for patient care. Assess, document, prioritize, and manage patients’ pain. Complete documentation and make calls after patient visits. Attend meetings and coordinate patient care. Arrange visit schedules for patients.
