Deliver high-quality, patient-centered care as a Home Health Registered Nurse. This opportunity calls for a compassionate RN eager to make a meaningful impact by delivering skilled nursing services in patient homes. Youll partner closely with patients, families, and interdisciplinary teams to drive positive outcomes and foster safe, effective, and holistic home environments.

Schedule:

• Monday Friday, 8:00 AM 5:00 PM
• Full-time, 13-week contract

Key Responsibilities:

• Conduct detailed OASIS assessments and develop individualized plans of care for home health patients.
• Deliver skilled nursing services such as wound care, medication administration, IV therapy, and chronic disease management.
• Assess patient conditions, identify changes, and promptly communicate findings to physicians and the healthcare team.
• Educate patients and caregivers about treatment, disease management, and medication regimens.
• Document all care accurately and in real time utilizing Home Care Home Base (HCHB).
• Coordinate care with other clinicians including therapists and medical social workers.
• Participate in case conferences and team meetings, maintaining compliance with regulatory and agency standards.
• Travel daily throughout an assigned territory with mileage reimbursement provided.

Qualifications:

• Active RN license in good standing in the state of practice
• At least 1 year of recent home health field RN experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Strong clinical decision-making and assessment abilities
• Reliable transportation, valid drivers license, and current auto insurance
• Excellent communication, organization, and independent time management skills
• BSN and experience managing 25-32 points per week preferred
• Familiarity with Medicare and compliance standards advantageous

$2,016 - $2,132/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
