Empowerment Case Manager for Adults with Disabilities
Independence Association, Inc.
Independence Association seeks a full-time case manager to join our adult case management team. You will assess needs, plan services, advocate, and coordinate resources to help individuals with intellectual disabilities or autism live full, inclusive lives. You will work with the person and their support network to develop yearly Person Centered Plans, document thoroughly, and participate in trainings and supervision. Travel with a valid driver’s license is required. #J-18808-Ljbffr Independence Association, Inc.
