Key Responsibilities

• Provide physical therapy services to patients in a home health setting
• Complete OASIS assessments accurately and in a timely manner
• Document all patient care activities using HCHB electronic medical records
• Participate in on-call rotation as required
• Meet productivity expectations of 30 points per week

Required Qualifications

• Active Physical Therapist (PT) license in the state of Ohio
• Minimum of 12 months of home health experience within the past 3 years
• Proficiency with HCHB electronic medical records system
• Experience completing OASIS documentation
• Ability to fulfill a 40-hour per week commitment

Preferred Experience / Skills

• Familiarity with home health regulatory and compliance standards
• Strong time management and independent patient care skills in a field-based environment
