- Evaluate patients' functional status and physical therapy needs.
- Collaborate with the physician to develop and update individualized therapy plans of care.
- Deliver physical therapy services within state-defined scope of practice and in alignment with the established plan of care.
- Coordinate care with nurses, physicians, and other members of the healthcare team.
- Observe, document, and report patient responses to treatment and any changes in condition.
- Provide therapeutic interventions to relieve pain, restore function, and maximize patient independence using accepted standards of practice.
- Educate patients, families, and caregivers on the plan of care, functional limitations, and progress toward goals.
- Complete patient visits as outlined in the Plan of Treatment and submit timely, accurate progress notes per agency policy.
- Conduct comprehensive evaluations and develop PT plans of care within state-specific guidelines.
- Report evaluation outcomes, goals, and projected frequency of care.
- Deliver care effectively in diverse home environments and with patients of all ages and backgrounds.
- Current Physical Therapy license in the state of practice.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and reliable transportation (or access to public transit).
- Previous Home Health experience.
