QUALIFICATIONS of the Licensed Practical Nurse / LPN:
A current unencumbered license to practice nursing in the state in which the center is located.
Proficiency in basic computer skills including an electronic health record, PCC is a plus!
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES AND STANDARDS of the Licensed Practical Nurse / LPN:
Demonstrates the ability to implement systems that address each resident’s needs. Having a broad scope of knowledge and experience to be able to address the needs of the medically complex as well as the mentally compromised residents.
Demonstrates the ability to assess the health care needs of the resident, make a nursing diagnosis, transition this into an interdisciplinary plan of care utilizing all resources available. Then is able to communicate this plan to all care providers in a way that ensures continuity of care for resident.
Demonstrates the ability to establish and maintain functional communications with staff, other disciplines, residents, families, medical staff and community.
Treats residents, staff and facility community with respect and dignity. Treating resident and facility property and information with honesty, confidentiality, and respect.
