Minimum Requirements Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA. Active Indiana Physical Therapy license required to start the assignment. BLS certification required from the American Heart Association or American Red Cross. Outpatient experience required. Candidate must have demonstrated competency evaluating and treating outpatient orthopedic and musculoskeletal conditions. No new graduates. Minimum of 1 year of clinical experience in outpatient settings preferred; specify as preferred when applicable. Experience with documentation, goal writing, letters of medical necessity, and charge entry using electronic medical records. Ability to supervise and delegate to therapy assistants and students in accordance with clinic policies and state practice act. Adherence to discipline-specific State Practice Act and maintenance of professional licensure in good standing.
Assignment Details Facility Type: Outpatient. Shift Pattern: 5x8 schedule. Duration: 13 week assignment. Positions Available: 1. Responsibilities include comprehensive evaluations, development and implementation of individualized treatment plans, timely documentation, and promoting patient and employee safety.
