This role provides patient centric care and is primarily responsible for nursing assessment and diagnosis, planning, implementation, and evaluation./ppThis position is 0.6 FTE, 24 hours/week./ph3Responsibilities/h3ulliAssesses physical, emotional, social, and spiritual needs and evaluates patients adaptation to health changes/liliDevelops a plan of care based on the patients disease, symptoms, and response to treatment/liliEducates patients, families, and caregivers on disease processes, treatment, side effects and symptom management/liliCoordinates with the interdisciplinary team to ensure continuity of care/liliRefers patients, families, and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc./liliImplements, evaluates, and modifies plan of care/liliIntegrates research and evidence-based knowledge into clinical practice/liliDirect Patient Care/liulliEducating patients and families: specific to rehabilitation services/liliTelephone and email symptom triage/liliEngage in meaningful patient follow-up and care coordination/liliex: post-procedure, medication side effects/liliNew patient orientation to the service line/liliCaregiver support/liliAssist with in-clinic procedures:/lili(set up, consent, time out, and patient positioning)/liliAssessment of needs for additional supportive care resources/liliPrescription refills and addressing medication questions/liliResult reviews/li/ulliIndirect Patient Care/liulliSupportive care referrals/liliFMLA paperwork, disability paperwork, Ferry passes/liliCoordination of outside clinical care/liliEnsuring continuity of care with outside providers/liliCollaboration with the research team for patients on trial/liliPlace physiatry return orders and Supportive Care referral orders as directed by MD/liliCoordinate responses to patient inquiries via MyChart and phone/liliCoordinate lab tests and notification of patients/liliCoordinate with Supportive Care and medical care teams as needed, across Fred Hutch sites/liliFacilitate external referrals for ongoing rehabilitation needs/liliManage Epic Rehab Medicine Clinical Support InBasket inquiries from patients, staff, or other providers/li/ulliCommunication and Teamwork/liulliAttend regular team meetings with the provider and nursing teams/liliCoordination of care with ancillary services/liliDelegation of tasks to appropriate team members/liliCoordinating patient schedules with the clinic team/liliCollaboration with infusion RN and other members of the care team to ensure patient needs are met/liliReal-time management and escalation of issues in the clinic/li/ulliProfessional Development and Clinic Engagement/liulliDocument all nursing care activities accurately and in a timely fashion./liliDemonstrate knowledge and understanding of patient privacy rights./liliActive participation in continuing education, departmental projects, and committees./li/ul/ulh3Qualifications/h3pstrongRequired:/strong/pulliGraduated from an accredited school of nursing/liliMust have a current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); and must upgrade to an MSL with primary state of residence, WA, within 60 days of hire.
