Care Manager MSW & Care Manager LCSW - Per Diem Providence Health and Services, Everett, WA This per diem role offers part‑time opportunities for licensed social workers to provide comprehensive patient‑centric care management services for at‑risk patients in an acute care environment. Care Manager LCSW The Inpatient Licensed Clinical Social Worker (LCSW) Care Manager delivers professional, comprehensive care management services, including assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance, and patient advocacy. The goal is to ensure continuity of care for vulnerable patients, identifying and addressing social, financial, cognitive/behavioral, or legal barriers to accessing necessary resources. Required Qualifications Master’s Degree in Social Work from an accredited Council on Social Work Education program Washington Clinical Independent Social Worker license upon hire At least 2 years of social work experience in any healthcare setting 1 year experience in an acute care setting or successful completion of a TIPs program or Case Management Orientation Program Preferred Qualifications Basic understanding of acute medical‑surgical healthcare terminology Prior case management experience Care Manager LCSW (Everett, WA) , Salary Range: Min: $49.41 Max: $76.38 Care Manager MSW The Inpatient Master of Social Work (MSW) Care Manager provides comprehensive, patient‑centric care management services, encompassing assessment and planning, coordination of care, discharge planning, documentation of interventions, regulatory compliance, and advocacy. The goal is to ensure continuity of care for vulnerable patients, identifying needed resources to address social, financial, cognitive/behavioral, or legal barriers to care access.
