Advance Practice Professional Nephrology Outpatient Moorefield, WV and Keyser, WV

Position Summary The Advanced Practice Professional (APP) coordinates and provides clinical care required for the comprehensive management of nephrology patients, including biological, social, and psychological assessments. The APP collaborates with medical specialty teams to develop a comprehensive care plan, reviews with clinical team members, participates in directing care, performs diagnostic/therapeutic procedures, and coordinates discharge planning as applicable. The APP also participates in education of residents, medical and nursing students, and provides healthcare services consistent with state law and professional licensing requirements. Minimum Qualifications – Nurse Practitioner Graduate of a CCNE accredited nursing program with a Master’s Degree in Nursing. National certification by a recognized APRN Certification Center. Licensure (or eligibility) as a Registered Nurse and Advanced Practice Registered Nurse in West Virginia. BLS certification required; additional Advanced Life Support training at departmental discretion. Advanced Cardiac Life Support (ACLS) at departmental discretion. Preferred Qualifications – Nurse Practitioner ACNP preferred; FNP considered. Acute care and/or nephrology experience preferred. Minimum Qualifications – Physician Assistant Bachelor’s Degree in Medical/Surgical PA program. Graduate of an ARC-PA approved physician assistant training program. Certification by the National Commission for Certification of Physician Assistants. Licensed or eligibility to be licensed by the WV Board of Medicine. BLS certification required; additional Advanced Life Support training at departmental discretion. Advanced Cardiac Life Support (ACLS) at departmental discretion. Preferred Qualifications – Physician Assistant Master’s level education in Medical/Surgical PA preferred. Acute care and/or nephrology experience preferred. Core Duties and Responsibilities Manages comprehensive and individualized care for nephrology patients and their families in the outpatient phase, post-discharge, and ambulatory setting. Completes daily rounds; consults and/or pre-procedure evaluations. Completes integrated assessments. Orders additional tests or consults as appropriate, following established guidelines and protocols. Performs and communicates results of diagnostic tests per clinical guidelines and scope of practice. Consults and communicates with clinic and hospital staff regarding abnormal findings. Performs succinct comprehensive clinical evaluations and documents findings in the medical record. Facilitates patient discharge by coordinating appropriate services and tests prior to discharge necessary for outpatient follow-up appointments. Develops a comprehensive individualized plan of care, including teaching, discharge planning, and referrals as necessary. Demonstrates expertise in caring for patients across the age continuum. Identifies unstable or complex patient care situations and facilitates appropriate intervention. Recognizes emergency situations and initiates effective emergency care. Evaluates patients and serves as coordinator for follow-up procedures and tests in ambulatory setting. Assists in the collection of outcome measures. Demonstrates critical thinking and diagnostic reasoning skills in clinical decision making. Demonstrates the ability to utilize equipment safely. Bases decisions on relevant scientific principles and established standards of care. Utilizes teaching/learning theory and appropriate teaching strategies in education of staff, students, patients, families, and community. Promotes a learning environment by responding to patient/family preferences and readiness to learn. Updates knowledge base throughout the year. Completes all identified clinical competencies. Serves as a resource person for staff caring for nephrology patients. Provides educational programs to professional and paraprofessional personnel in area of expertise. Participates in community programs and distribution of literature as appropriate. Documents the plan of care, interventions, and evaluations clearly and concisely for every patient visit. Collaborates with leaders and staff to achieve patient outcome-based improvements in care. Provides leadership in the development and implementation of changes in nephrology practice that positively impact outcomes. Works collaboratively with medical and nursing staff to facilitate creative problem solving and provide professional support. Participates and initiates interdisciplinary approaches to patient care, program development, and education, serving as a professional role model. Promotes and practices innovation of the expanded role in delivery of care to nephrology patients and families throughout outpatient hospitalization and ambulatory setting. Participates and provides leadership for committees and task forces. Documents hours and submits billing reports as appropriate. Reviews and interprets nephrology data from registries and reports, and recommends/implements changes as necessary. Participates in clinical trials and research studies with the Principal Investigator. Assists faculty, administrators, and directors in the collection of outcome measures. Physical Requirements Heavy/Hard Work: Requires strength and/or stamina to lift, move, stoop, reach, position patients, stand, walk, carry materials and equipment weighing 40+ pounds. Working Environment Working closely with others. Protracted or irregular hours. Working around biohazards and infectious diseases. Working around deceased patients. Working with hands in water. Electrical hazards associated with patient care equipment. Skills and Abilities Proficiency with computers. Strong communication skills.
