This opportunity offers the autonomy of providing one-on-one patient care while helping individuals regain mobility, improve function, and remain safely in their homes. Job Responsibilities Perform comprehensive physical therapy evaluations in patients' homes. Develop individualized treatment plans based on each patient's goals and functional needs. Treat patients recovering from orthopedic procedures, neurological conditions, cardiopulmonary diagnoses, and generalized weakness. Educate patients and caregivers on home exercise programs, mobility, safety, and fall prevention. Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers. Complete timely and accurate documentation using Homecare Homebase (HCHB) . Travel throughout the assigned coverage area while providing exceptional patient-centered care. Qualifications Active Tennessee Physical Therapist license (or ability to obtain before start) Minimum of 1 year of recent Home Health experience OASIS documentation experience required Homecare Homebase (HCHB) experience preferred Current BLS certification Valid driver's license, reliable transportation, and current auto insurance Strong communication and organizational skills About Amedisys Home Health Amedisys is one of the nation's leading providers of home health, hospice, and high-acuity care services.
