Bilingual Senior Care Community Health Advocate

Suvida Healthcare is seeking a Guia to coordinate care for Medicare-eligible Hispanic seniors. You will engage patients and families, collaborate with a multidisciplinary team, and connect them to needed medical and social services to improve health outcomes.

Ideal candidates have 4–5 years in healthcare, CHW certification or higher, and strong communication and cultural competency. The role involves in-home and community outreach with a supportive team and competitive pay.
