Paradigm at Faith Memorial is a skilled nursing facility seeking an experienced, compassionate and dedicated Wound Care Nurse to join the Paradigm Healthcare family!Sat&Sun 8am-5pmJob DutiesProvide direct nursing care in accordance with the nurse practice act, federal and state regulations, facility policies and procedures, and prudent nursing judgmentPlan, develop, organize, implement, evaluate, and direct the delivery of a resident’s plan of care in accordance with current physician’s orders, rules, regulations, and guidelines that govern the nursing care facilitiesHave knowledge of and utilize of the facility’s written policies and procedures that govern the day-to-day functions of the nursing service departmentUtilize established methods for coordination and communication of nursing services with other resident services to ensure the continuity of the residents’ total regimen of careParticipate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directedConduct themselves in a professional manner in compliance with facility policiesAgree not to disclose assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the AdministratorAgree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the AdministratorReport any known or suspected unauthorized attempt to access facility’s information system.Participate in developing, evaluating, revising, and implementing a plan of care for each resident to achieve and maintain the highest practicable level of wellbeing possibleProvide education to residents/families/caregivers when the need arises, or as directed by Administration and/or physicianProvides prompt emergency support measures, including CPR, in accordance with physician orders, resident/family wishes, advance directives, and facility policies and procedures.Provide care in a professional/courteous manner by ensuring the resident is approached in a kind, gentle and friendly mannerProvide care in a manner that respect for the resident's dignity and privacy is consistently upheldEnsure that nursing interventions are performed in a timely manner. If there must be a delay, explanations for delays in answers/responses are providedDevelop daily work assignments and unit assignments, and/or assist nursing supervisory staff in completing and performing such tasksMake frequent rounds of the assigned unit to ensure that all nursing service personnel are performing their work assignments in accordance with acceptable nursing standardsProvide care in a manner that is courteous and respects the residents’ dignity and privacyProvide complaint/grievance reports to the Administrator as required or as may be necessaryReport occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the facility’s policies and procedures governing accidents and incidentsReport known or suspected incidents of fraud, abuse, neglect, and/or misappropriation to the AdministratorEnsure that departmental computer workstations left unattended are properly logged off or the password protected automatic screensaver activates within established facility policy guidelinesRefrain from disclosing assigned user ID code and password for accessing resident/facility information and promptly report suspected or known violations of such disclosure to the AdministratorRefrain from disclosing resident protected health information and promptly report suspected or known violations of such disclosure to the AdministratorReport any known or suspected unauthorized attempt to access facility’s information systemBenefitsHealth insurancePaid time offDental insuranceVision insuranceLife insuranceReferral programProfessional development assistanceRequired Education and ExperienceMust possess a current, unencumbered, active license to practice as a Registered Nurse or Licensed Vocational Nurse in the StateAn Associate Degree in Nursing is required2 years’ experience in Long-Term Care experience preferredSix (6) months experience in rehabilitative and restorative nursing practices preferredStrong utilization knowledge of Point Click Care preferredComputer literacy skills (Microsoft Outlook, Microsoft Office, Internet).Paradigm Healthcare was founded on the belief that by empowering each other, we can achieve more and provide better care to every patient we serve.
