Position Overview

A growing home health organization is seeking a compassionate Physical Therapist to provide in-home rehabilitation services throughout Catawba and Alexander Counties. This role offers the flexibility to manage your own schedule while making a meaningful impact by helping patients regain mobility, improve function, and maintain independence in the comfort of their own homes.

Whether you're an experienced home health therapist or a new graduate looking to begin your career, you'll receive the support and resources needed to succeed.

Responsibilities

• Provide skilled physical therapy services in patients' homes
• Develop and implement individualized treatment plans
• Evaluate patient progress and modify treatment as needed
• Educate patients and caregivers on exercises, mobility, and safety
• Collaborate with physicians and interdisciplinary care teams
• Complete accurate and timely clinical documentation

Requirements

• Active North Carolina Physical Therapist license
• Current CPR certification
• Valid driver's license and reliable transportation
• New graduates welcome to apply
• Home Health experience is not required
• Excellent communication and patient education skills
• Commitment to delivering compassionate, patient-centered care
