Hospice Medical Social Work Liaison — Part-Time Care Coordinator

Ohio’s Hospice seeks a Social Work Liaison to partner with hospital discharge planners, physicians, and the palliative care team to assist with admitting patients to the Ohio’s Hospice program and to coordinate safe transfers to ECF, home or Hospice Care Center. Based at the inpatient unit of Ohio State University Wexner Medical Center, this part-time role requires travel to the inpatient care center at Licking Memorial Hospital; hours are 8 AM–5 PM. #J-18808-Ljbffr Ohio's Hospice, Inc.
