Description

• Conduct evaluations for patients in both inpatient and outpatient environments, creating tailored treatment strategies.
• Deliver physical therapy treatments aimed at improving mobility, strength, balance, and functional abilities.
• Work in synergy with nurses, physicians, case managers, and other rehabilitation professionals to optimize patient outcomes.
• Provide education to patients and their families on therapeutic goals, planning for discharge, and home exercise regimens.
• Document evaluations, treatments, and progress per facility standards and regulatory requirements.
• Engage in care team meetings and interdisciplinary discussions to ensure seamless patient care.

Education

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution.

Certifications

• Valid state licensure as a Physical Therapist.
• CPR certification is preferred.
• Additional specialized certifications (e.g., in geriatrics or neurology) are advantageous but not required.

Skills

• Prior experience is highly valued.
• Ability to manage patients with intricate medical histories and mobility challenges.
• Excellent communication and teamwork capabilities.
• Familiarity with digital charting and clinical documentation software.
